For example, interleukin 6 (IL-6) can induce apo(a) synthesis by triggering the promoter region on the LPA gene, and IL-6 blockade with tocilizumab significantly reduced Lp(a) levels via transcriptional suppression of the LPA gene. |

Recent evidence suggests that this IL-6-mediated induction is negatively regulated by peroxisome proliferator-activated receptor gamma (PPARg), as genetic and in vitro studies have shown that PPARg suppresses IL-6-driven LPA transcription. |

Assembly of Lp(a) occurs in a two-step process: first, there is a non-covalent interaction between the KIV-5 and KIV-8 domains of apo(a) and the N terminus of apoB. Then a covalent disulfide bond forms between the unpaired KIV-9 cysteine on apo(a) and the unpaired cysteine 4326 in apoB, yielding a mature Lp(a) particle. Notably, recent research has led to the development of small molecule inhibitors such as muvalaplin that can prevent the initial apo(a)-apoB interaction, thus blocking Lp(a) particle assembly. |
